1. The Menstrual Cycle Explained
The menstrual cycle is a monthly hormonal sequence that prepares the body for potential pregnancy. It is measured from the first day of one period to the first day of the next. While the average cycle is 28 days, a healthy cycle can range from 21 to 35 days, and many women experience natural variation of several days between cycles.
The cycle is divided into two main phases, separated by ovulation:
- Follicular phase (days 1 to ovulation): Oestrogen rises, causing the uterine lining to thicken and a follicle in the ovary to mature.
- Luteal phase (ovulation to next period): After ovulation, the empty follicle becomes the corpus luteum and produces progesterone to maintain the uterine lining. This phase is typically fixed at 12–16 days.
2. When Does Ovulation Occur?
Ovulation — the release of a mature egg from the ovary — is triggered by a surge in luteinising hormone (LH), which occurs approximately 24–36 hours beforehand. In a textbook 28-day cycle, ovulation occurs around day 14. However, because the follicular phase varies, ovulation can occur anywhere from day 10 to day 21 in women with typical cycle lengths.
The egg, once released, is only viable for fertilisation for 12–24 hours. However, sperm can survive in the female reproductive tract for up to five days, which is why the fertile window extends several days before ovulation.
3. Your Fertile Window
The fertile window is the period during which unprotected intercourse can result in pregnancy. Because sperm can survive up to five days inside the body, and because the egg is viable for up to 24 hours, the fertile window spans approximately six days: the five days before ovulation, plus the day of ovulation itself.
The two days immediately before ovulation and ovulation day itself carry the highest probability of conception.

4. Physical Signs of Ovulation
Several physical changes accompany ovulation that can be used to identify the fertile window:
Cervical Mucus Changes
As ovulation approaches, cervical mucus changes from dry or creamy to clear, stretchy, and slippery — often described as resembling raw egg white. This "fertile-quality" mucus helps sperm survive and travel through the cervix. The presence of this mucus is one of the most reliable signs that ovulation is approaching.
Basal Body Temperature (BBT)
After ovulation, progesterone causes a slight but measurable rise in resting (basal) body temperature — typically 0.2–0.5°C above the pre-ovulatory baseline. This rise confirms that ovulation has occurred, though it does not predict it. Tracking BBT over several cycles can help identify your typical ovulation timing in retrospect.
Ovulation Pain (Mittelschmerz)
Approximately 20% of women experience a mild one-sided pelvic ache or twinge at or around ovulation — known as mittelschmerz (German for "middle pain"). This can occur on either side and typically lasts from a few minutes to a few hours.
LH Surge (Detected by OPKs)
Ovulation predictor kits (OPKs) detect the LH surge in urine 24–36 hours before ovulation. A positive OPK indicates that ovulation is imminent and that the next 1–2 days are the most fertile of the cycle.
5. Methods for Tracking Your Fertile Window
- Calendar / cycle tracking: Suitable for women with regular cycles. Track at least 6 cycles to identify your typical ovulation window. Least accurate method.
- Cervical mucus monitoring: Observe and record changes daily. More accurate than calendar tracking alone.
- BBT charting: Take temperature first thing every morning before getting up. Reveals ovulation in retrospect — best used alongside other methods.
- Ovulation predictor kits (OPKs): Urine tests that detect LH surge. Highly accurate for predicting ovulation 24–36 hours ahead.
- Fertility monitors: Digital devices (e.g. Clearblue) that track both LH and oestrogen, identifying a wider fertile window. Most accurate consumer option.
6. Irregular Cycles and Fertility Tracking
Women with irregular cycles — those that vary by more than 7–9 days month to month — face greater challenges predicting ovulation from calendar tracking alone. Conditions such as polycystic ovary syndrome (PCOS) can cause irregular or absent ovulation. If you have irregular cycles and have been trying to conceive for 12 months (or 6 months if over 35), speak with your GP, who can arrange investigations including a day 21 progesterone test to confirm ovulation.
7. Common Fertility Myths
- Myth: You always ovulate on day 14. Only true for women with exactly 28-day cycles. Ovulation day varies widely.
- Myth: You can't get pregnant during your period. While unlikely, it's possible — particularly if you have a short cycle and long period, as sperm can survive until ovulation.
- Myth: Having sex every day maximises chances. Research suggests every 1–2 days during the fertile window is optimal. Daily intercourse may lower sperm count in some men.
